<![CDATA[ Licensed to Practice begins with an 1891 shooting in Wheeling, West Virginia, that left one doctor dead and another on trial for his life. Formerly close friends, the doctors had fallen out over the issue of medical licensing. Historian James C. Mohr calls the murder "a sorry personal consequence of the far larger and historically significant battle among West Virginia’s physicians over the future of their profession."

Through most of the nineteenth century, anyone could call themselves a doctor and could practice medicine on whatever basis they wished. But an 1889 U.S. Supreme Court case, Dent v. West Virginia, effectively transformed medical practice from an unregulated occupation to a legally recognized profession. The political and legal battles that led up to the decision were unusually bitter—especially among physicians themselves—and the outcome was far from a foregone conclusion.

So-called Regular physicians wanted to impose their own standards on the wide-open medical marketplace in which they and such non-Regulars as Thomsonians, Botanics, Hydropaths, Homeopaths, and Eclectics competed. The Regulars achieved their goal by persuading the state legislature to make it a crime for anyone to practice without a license from the Board of Health, which they controlled. When the high court approved that arrangement—despite constitutional challenges—the licensing precedents established in West Virginia became the bedrock on which the modern American medical structure was built. And those precedents would have profound implications. Thus does Dent, a little-known Supreme Court case, influence how Americans receive health care more than a hundred years after the fact.]]>

In his Metaphysics, Aristotle observed “we understand best those things we see grow from their very beginnings.” Thus one important role of historians is to tell us about those beginnings. In “Licensed to Practice” the distinguished historian James Mohr tells the remarkable story of how the practice of state licensing physicians began.

It is easy to see how the public’s health is under threat if just anyone can hang out a shingle and open a medical practice. Yet, this was precisely the case in the United States throughout much of the 19th century. Few disagreed that only qualified people should be allowed to practice medicine, but controversy erupted over the joint questions of what constitutes being qualified, and who should decide. As one might suspect the controversy included approximately equal parts of science and politics.

Professor Mohr tells the story of how the movement to license physicians grew strong among a minority of physicians in West Virginia who through dint of cleverness and mighty effort convinced the West Virginia legislature to take on this task as a small part of a much greater effort to improve public health. Those whose approach to practicing medicine differed from that which was adopted as required to be licensed understandably objected strenuously. There were a substantial number of individuals, now outsiders to the guild of medicine, who didn’t seem to understand the logic being followed – of course it is always difficult to get someone to understand something if his paycheck requires that he not understand it. Their objections ended up in a legal challenge to the West Virginia statute that ended before the US Supreme Court, which in 1889 (Dent vs.

The new release of CPR book is really amazing, long chapters have been re-edited and the material is very up to date. The papers are of good quality.

The only disadvantage is that lww wanted to retain the rights of photocopy, so some chapters are only available on the point website without being included in the book. This is a real disadvantage for students or readers who want to have the hard copy in hand!!!

This is surprisingly, one fascinating and important read that spins the historical reality of pathogenic disease with a well crafted story regarding the plight of a society facing a treacherous epidemic. Combining an in-depth view regarding the indefatigable energy and brilliance of Dr. John Snow in his quest to solve the deadliest outbreak of cholera in the history of London, with the history of epidemic plagues, `The Ghost Map: The Story of London's Most Terrifying Epidemic- And How it Changed Cities, Science, and The Modern World' provided me with one page-turning, gripping historical tale that also provided further insight into the plight free societies face today in lieu of the possabilities of biological or chemical attacks on innocent people.

When I was recommended to read Steven Johnson's book, it was not for the sake of diving into a good read, but rather to `browse' through it for further insight on the origins of water contamination and how, thru these origins, terrorist could look at contamination for horrific purposes. As a writer with an interest in international affairs, and a tendency to use fiction storytelling to share my views, I opened Steven Johnson's book and within pages was completely hooked on this extraordinarily written, well researched tell all of the London epidemic of cholera that killed so many lives.

With reflection on how science viewed pathogenic outbreaks during the midpoint of the 19th Century, it was startling to find that there really existed a classification system that gave all sorts of bizarre reasons why a disease would spread, including a weight based upon wealth and financial disposition! We sure have come a long way . . . or have we?
The Ghost Map uses the Cholera outbreak in London in 1854 to weave together a compelling story of science, demographics and superstition. Rather than just engaging in a straightforward narrative about the outbreak, Johnson dives into a truly interesting analysis of the fear with which people viewed city living in the 1850s. Back then, nobody knew if a city of 2 million people, like London, would simply crumble under its own social weight. He also digs deeply into the science and medicine of the time (or lack thereof) and how it treated an outbreak like 1854's. In addition, he lays out the story of John Snow doing true scientific work, finding the real cause of the outbreak, winning some important converts, failing to win others, but ultimately saving many lives.

What makes the book so good is the way it places you into the mind of someone living in London in 1854 and making you understand why it was so hard for them to accept the true cause of the disease when it seems so obvious to us today. That experience makes a thoughtful reader wonder what things we take for granted today that will seem so obviously wrong in 150 years.

The book stays at four stars, not five, for several reasons. First off, the actual namesake of the book, The Ghost Map, is little more than a tacked-on afterthought at the conclusion of the book. It's interesting, but more of a post-script than anything else, and certainly not appropriate as the title of the book - somebody must have thought it sounded like it would sell books. No worries though, the book it sells is a good one.

Also, Johnson goes on some odd tangents at the end of the book talking about city life and trying to tie internet technology back to the work Snow did. It's a reach and not terribly relevant.

            This is another one of those must have books on your reference bookshelf if you deal with anything MSK. If you are a chiropractor, physical therapist, manual therapist, osteopathic physician, medical physician, athletic trainer, or personal trainer then you need to read this book. I find it amazing how many post-graduate education courses cite this as a source for their work or a recommendation as to it's use in your practice in order for you to "sharpen the saw". As a Sports Chiropractor I find myself going back to this reference frequently to explain to patients what is going on as well as using the chains as my guidelines for taking an athlete for performance.

            Fascia (connective tissue in our bodies) has played a significant role in the way we move. Thomas W. Myers explores human movement and fascia in relation to health professionals, dancers, and athletes in Anatomy Trains: Myofascial Meridians for Manuael and Movement Therapists. He explores "our largest sensory organ" in a way that laymen will understand clearly.

            Readers of Myers' text have described it as "revolutionary" in its approach to physical therapy. It certainly isn't the first time these ideas have been put to print, but its certainly good information to propagate. My interest in the topic led me to Human Movement Potential: Its Ideokinetic Facilitation, which was first published in 1974 by Lulu Sweigard who was an authority on posture and movement. Myers' modern book has an easy-to-read layout that gathers all the general ideas and explains them simply. Illustrations are also helpful in demonstrating the breakdown of human anatomy to the cellular level.

            Topics like gait and fitness are connected in more ways than one. It is Myers' hope that health practitioners and athletes will apply the techniques and treatments in this book as they reconsider the concept of movement defined as a "straight line." Ultimately, Myers breaks down the traditional notions of what makes a body, in order to inspire new generations who will think about body movement strategies for a better future.

            This book will fit well on any one's shelf related to movement. This includes massage therapists, yoga instructors, osteopaths, and even those who train in athletics and dancing.

            In his Metaphysics, Aristotle observed “we understand best those things we see grow from their very beginnings.” Thus one important role of historians is to tell us about those beginnings. In “Licensed to Practice” the distinguished historian James Mohr tells the remarkable story of how the practice of state licensing physicians began.

            It is easy to see how the public’s health is under threat if just anyone can hang out a shingle and open a medical practice. Yet, this was precisely the case in the United States throughout much of the 19th century. Few disagreed that only qualified people should be allowed to practice medicine, but controversy erupted over the joint questions of what constitutes being qualified, and who should decide. As one might suspect the controversy included approximately equal parts of science and politics.

            Professor Mohr tells the story of how the movement to license physicians grew strong among a minority of physicians in West Virginia who through dint of cleverness and mighty effort convinced the West Virginia legislature to take on this task as a small part of a much greater effort to improve public health. Those whose approach to practicing medicine differed from that which was adopted as required to be licensed understandably objected strenuously. There were a substantial number of individuals, now outsiders to the guild of medicine, who didn’t seem to understand the logic being followed – of course it is always difficult to get someone to understand something if his paycheck requires that he not understand it.             I have been teaching social science research methods since 1978. As a result, I believe I have reviewed most of the textbooks in the area. As a result, I feel I can make a couple of remarks regarding the work of DePoy and Gitlin.
            First and foremost, this is a textbook. It was specifically designed for students in allied health professional studies and human service curricula. Compared to most textbooks, it is NOT dry writing. Clearly, the authors have a flare for words.
            Second, the primary strength of this text is its evenhandedness. More so than any other text, DePoy and Gitlin address both the qualitative and quantitative methodologies. No text completes this task as effectively and as efficiently.
            When Ph.D. students (within my doctoral cohort) were being educated, they shared an unsavory learning experience. Most of us were taught to become number crunchers. The value of qualitative research was downplayed. In some cases we were specifically taught that qualitative research methodologies were hampering the image and progress of the social sciences. Thus, qualitative techniques were portrayed as a strategy employed by professors to engage the interest of undergraduate students. Qualitative methods were often adopted as a "play time." The possible exception for my observation is anthropology. However, even doctoral educators in anthropology noted the value of number crunching.
            DePoy and Gitlin offer a doorway to turn our thinking around. They contend that the research question must drive the research method - not the other way around. During my education (Baccalaureate, Masters, and Ph.D.), I was taught to design research questions that could be answered by employing regression analysis or a variation of it.

            This is surprisingly, one fascinating and important read that spins the historical reality of pathogenic disease with a well crafted story regarding the plight of a society facing a treacherous epidemic. Combining an in-depth view regarding the indefatigable energy and brilliance of Dr. John Snow in his quest to solve the deadliest outbreak of cholera in the history of London, with the history of epidemic plagues, `The Ghost Map: The Story of London's Most Terrifying Epidemic- And How it Changed Cities, Science, and The Modern World' provided me with one page-turning, gripping historical tale that also provided further insight into the plight free societies face today in lieu of the possabilities of biological or chemical attacks on innocent people.

            When I was recommended to read Steven Johnson's book, it was not for the sake of diving into a good read, but rather to `browse' through it for further insight on the origins of water contamination and how, thru these origins, terrorist could look at contamination for horrific purposes. As a writer with an interest in international affairs, and a tendency to use fiction storytelling to share my views, I opened Steven Johnson's book and within pages was completely hooked on this extraordinarily written, well researched tell all of the London epidemic of cholera that killed so many lives.

            With reflection on how science viewed pathogenic outbreaks during the midpoint of the 19th Century, it was startling to find that there really existed a classification system that gave all sorts of bizarre reasons why a disease would spread, including a weight based upon wealth and financial disposition! We sure have come a long way . . . or have we?
            The Ghost Map uses the Cholera outbreak in London in 1854 to weave together a compelling story of science, demographics and superstition. Rather than just engaging in a straightforward narrative about the outbreak, Johnson dives into a truly interesting analysis of the fear with which people viewed city living in the 1850s. Back then, nobody knew if a city of 2 million people, like London, would simply crumble under its own social weight. He also digs deeply into the science and medicine of the time (or lack thereof) and how it treated an outbreak like 1854's. In addition, he lays out the story of John Snow doing true scientific work, finding the real cause of the outbreak, winning some important converts, failing to win others, but ultimately saving many lives.

            What makes the book so good is the way it places you into the mind of someone living in London in 1854 and making you understand why it was so hard for them to accept the true cause of the disease when it seems so obvious to us today. That experience makes a thoughtful reader wonder what things we take for granted today that will seem so obviously wrong in 150 years.

            The book stays at four stars, not five, for several reasons. First off, the actual namesake of the book, The Ghost Map, is little more than a tacked-on afterthought at the conclusion of the book. It's interesting, but more of a post-script than anything else, and certainly not appropriate as the title of the book - somebody must have thought it sounded like it would sell books. No worries though, the book it sells is a good one.

            Also, Johnson goes on some odd tangents at the end of the book talking about city life and trying to tie internet technology back to the work Snow did. It's a reach and not terribly relevant.

            Stahl's book is extremely useful, very helpful for the actual work of deciding which Rx to use, how to use it, and what to be cafeful about. The only drawback to this otherwise really excellent manual, is that the author does not include the anti-parkinsonian drugs that we sometimes use to treat EPS side effects.

            I am a primary care NP. I encounter patients who are already on a lot of psych meds and I also prescribe meds for depression, anxiety, etc. I have found this book to be absolutely essential. It covers each drug individually, and covers dosing, side effects, interactions, special populations, etc. in a complete yet concise manner. I recently had a patient who wanted to stop taking Cymbalta, which had been prescribed for fibromyalgia. I needed to quickly know approximately how long a taper I needed to design for her, and the information was in this book, as opposed to the ambiguous language of the manufacturer's prescribing guide.

            There is one problem for me, though. I immediately realized that this is a book I want in my office *and* at home. I want it at home -- either in print or digital form -- for those times when I am on call and say, a patient calls on a Saturday with complaints of side effects of one of these medications. Or maybe they want to stop the drug, like my patient on Cymbalta. But when I went to the online website (sorry, Amazon probably won't let me link to it), the publisher wants $195/year for a subscription to Stahl's Essential Psychopharmacology online. While Essential Pharmacology consists of more than just the Prescriber's Guide, I don't really need that. I just want to buy this book. There is no Kindle version of this book, and there is no smartphone app. So I am left to decide whether I want to buy another print version or maybe buy a cheaper print version of an earlier edition. The $195 online access is just way too expensive. The publisher needs to get with the times and offer reasonably-priced digital/online/app versions of this book.

            For a beginning biochemistry text they make simple concepts overly complicated and wordy. Important concepts are not easily taken out of the text and they often refer to information in chapters that are much later in the text. The problems are the end of each chapter are extremely confusing and require you to do a search and find to figure out what tables you should refer to since the problem often does not tell you where you will find the information. Sometimes when a problem does refer to a table you go to look for the table and realize that they were all renumbered in the revised edition (though they forgot to change the table references in the problems). There are also numerous spelling and grammatical errors. Nothing about this text is straight forward or easy to understand.

            In all my science courses this is probably the worst book I have ever encountered and have resorted to using other sources for learning most concepts as this book does not explain things clearly.

            I really, really hated this book and it doesn't help that my professor used it like a crutch. I could read and write notes about a chapter, get to the end of the chapter questions, and not have a clue on how to answer the questions. There also aren't any examples throughout the chapter on how to solve the problems. All the answers are at the end of the book, but it really didn't help because there isn't anyway to find out where they got that answer.

            Sheldon Watts’ Epidemics and History: Disease, Power and Imperialism argues that diseases and disease Constructs were used as tools to reinforce imperialist power. Watts defines a disease Construct as a perception “arrived at through a complex of cultural filters… [that] determined what – if anything – should be done in an attempt to limit disease transmission.” (p. xv) Positing that diseases spread as an “unintended consequence of [imperialistic] Development” and were often used to justify social policing, he concludes that epidemics were utilized as a route to power legitimization. (p. xiv)

            According to Watts, those in power used disease Constructs as tool of social discrimination and ostracization. He posits “accusations [of disease] were merely a ploy to put inconvenient people out of the way.” (p. 48) For example, the Construct of leprosy justified the imprisonment of those on society’s periphery to leprosaria during the Great Leper Hunt, and Watts concludes that a majority of those imprisoned were erroneously condemned. Later archeological studies have shown that the bones of the condemned had no lesions consistent with Hansen’s disease. The bubonic plague also legitimized elitist execution and consolidation of power. The plague allowed for “the growth of interventionist agencies” which violently repressed Jews, regulated burials, and mandated then new quarantine procedures. (p. 24)

            Watts also argues that disease Constructs created jobs and were used to justify imperial policies. During the period of The Great Leper Hunt, the clergy was expanding beyond the needs of existing congregations. Conveniently, the growing number of leprosaria “provided jobs for thousands of priests who otherwise would have no altars before which to perform their sacred rites.” (p.
