I've debated for several days after reading "On Killing" whether to post a review or not. I have tremendous respect for the author and his professional credentials but must disagree with his thesis and especially his use of two sources in particular. The author is a devotee of S.L.A. Marshall as were many until two works in the late 1980s cast serious doubts on Marshall's methodology and even his personal character. This blew open while I was a graduate student, specializing in military history and therefore became a topic of intense debate within my circle of fellow students and professors, especially my mentor, who was a British Commando in WWII. The second source I would debate is some of the information the author took from Paddy Griffith's works on the American Civil War.

The underlying thesis of "On Killing" is that mankind is instinctively hard wired Not To Kill. How I wish that was true, and yet our bloody record across recorded history and plenty of evidence even prior to recorded history shows the exact opposite. We are, by instinct "killer angels." Read "War Before Civilization" as but one counter argument. But directly to my concern about the author's sources. "SLAM" Marshall's reputation was built on alleged interviews, hundreds of them, immediately after combat during WWII in which he asserts that at least 75% of combat infantry never fired their weapons, thereby proving that soldiers, at least American soldiers abhor killing and try to avoid doing so even at the risk of their lives.
I cannot recommend this book to anyone. I hoped I would find in it a well-documented, well-thought-out treatment of the subject matter. To my surprise, I found instead a sensationalized polemic advocating the censorship of violent video games.

The author was unconvincing in his arguments. It is clear from his cherry-picking of statistics that he wants us to believe that we live in a society of ever-increasing violence. Unfortunately for Grossman, US Department of Justice statistics contradict this assertion. According to DOJ numbers easily found through a Google search, violent crime rates (including homicide)in America skyrocketed from about 1960 to the early 1990s, but have been falling steadily since then. Would anyone argue that the use of violent video games in the US is falling steadily as well? He also fails to mention that certain societies with arguably even more violent video games than the US have much lower rates of violent crime than we do, for example Japan.

The author seems to rely heavily on a few secondary sources, particularly John Keegan's Face of Battle and Richard Holmes' Acts of War. His few primary sources include articles from Soldier of Fortune magazine; he appears to take them at face value that they are true, accurate first-person accounts of combat experiences. He claims that he himself conducted several hundred interviews of combat veterans, but didn't seem to use their accounts as sources.

His personal bias in on display here, but he seems unaware of it. He lionizes the American soldier. I served as an American soldier for two decades before retiring in 2001. I came to view my fellow soldiers as ordinary fallen beings sometimes performing unpleasant tasks in unpleasant places.

I received an advanced reading copy of this from the publisher via Goodread's First-reads Giveaway program.

This popular science book is a broad overview of genetic and epigenetic inheritance, basically exactly what the subtitle says. The introduction oversells the the epigenetic focus (how life experience or environment can lead the changes in DNA that are not strictly sequence-based) because the majority of the book does stay within the realms of traditional sequence-based inherited genetic variation. Moreover, given Moalem's specialty, the focus is not so much on inheritance itself, nor even the specific mechanisms of inheritance.

Instead this book really comes down to these ideas: 1) There are a lot of genetic disorders. 2) Individually these disorders are often rare. 3) It is fairly likely that an given individual though will have some kind of disorder. In other words, everyone is unique; most all of us have unique rare disorders of some severity or another. The truth of this may surprise some, as may the implications: namely that any health advisories are tailored for the 'average population'. But no one is average. So not everyone can take the same amounts of medication. Eating high amounts of fat may be great for some people. Eating any fruits may be really bad for someone else. Running is good exercise for your spouse, it might give you a heart-attack, etc.

"Inheritance" thereby sweeps across a wide realm of human genetic variation, threading topics together under common themes. Moalem avoids getting bogged down into a lot of detail, making this book of greatest interest to the general public with medical interests, or those in particular who find medical anomalies interesting.

I'm a little taken aback to be the first reviewer to weigh in on Alex Perry's excellent book a full three months after its hardcover publication, but I'm honored nevertheless. Perry's book follows successful businessman Ray Chambers (he's the 'ray' in Wesray - together with ex-Treasury Secretary, William E. Simon, they were pioneers in the private equity field) in his role as United Nations Secretary-General's Special Envoy for Malaria. The co-founder of the NGO Malaria No More ('MNM'), Perry details Chambers' "ideas of enlightened self-interest" brought to bear on a problem of global - and seemingly insurmountable - scale.

The story Perry relates is of MNM's efforts to end malaria deaths by 2015. To do so, the group sets an aggressive goal to finance and distribute hundreds of millions of bednets by the end of 2010. Chambers and malaria are a good match. By all accounts, malaria is not a medical problem, it's a logistics problem. By spraying insecticides and blanketing malarial districts in bednets, experts know that they can interrupt the cycle of the disease. Applied assiduously, the effects can be immediate and dramatic.

What better agent than Ray Chambers to get this done? Chambers believed that "[I]nequality was unintelligent. Poverty didn't just hurt the poor. It hurt everyone." Others reached the same conclusion. The Bill and Melinda Gates Foundation - seen in these pages often intersecting with Chambers and team - come from the same viewpoint. But the surprise of the story is that so do corporate actors like ExxonMobil and AngloGold Ashanti.

About the Author

Jessica Keyes is president of New Art Technologies, Inc., a high technology and management consultancy and development firm started in New York in 1989. Keyes has given seminars for such prestigious universities as Carnegie Mellon, Boston University, University of Illinois, James Madison University, and San Francisco State University. She is a frequent keynote speaker on the topics of competitive strategy and productivity and quality.

She is former advisor for DataPro, McGraw-Hill’s computer research arm, as well as a member of the Sprint Business Council. Keyes is also a founding Board of Director member of the New York Software Industry Association. She completed a 2-year term on the Mayor of New York City’s Small Business Advisory Council. She currently facilitates doctoral and other courses for the University of Phoenix and is a member of the Faculty Council for the College of Information Systems and Technology. She has been the editor for WG&L’s Handbook of eBusiness and CRC Press’ Systems Development Management and Information Management.

Prior to founding New Art, Keyes was managing director of R&D for the New York Stock Exchange and has been an officer with Swiss Bank Co. and Banker’s Trust, both in New York City. She holds a master of business administration from New York University and a doctorate in management.

Surgeon and MacArthur fellow Gawande applies his gift for dulcet prose to medical and ethical dilemmas in this collection of 12 original and previously published essays adapted from the New England Journal of Medicine and the New Yorker. If his 2002 collection, Complications, addressed the unfathomable intractability of the body, this is largely about how we erect barriers to seamless and thorough care. Doctors know they should wash their hands more often to avoid bacterial transfer in the ward, but once a minute does seem extreme. Using chaperones for breast exams seems a fine idea, but it does make situations awkward. "The social dimension turns out to be as essential as the scientific," Gawande writes—a conclusion that could serve as a thumbnail summary of his entire output. The heart of the book are the chapters "What Doctors Owe," about the U.S.'s blinkered malpractice system, and "Piecework," about what doctors earn. Cheerier, paradoxically, are the chapters involving polio and cystic fibrosis, featuring Dr. Pankaj Bhatnagar and Dr. Warren Warwick, two remarkable men who have been able to catapult their humanity into their work rather than constantly stumble over it. Indeed, one suspects that once we cure the ills of the health care system, we'll look back and see that Gawande's writings were part of the story. (Apr.)

Critics appreciated his candor, his sly sense of humor, and his skill in examining difficult issues from many perspectives. He conveys his message—that doctors are only human and therefore must always be diligent and resourceful in fulfilling their duties—in clear, confident prose. Most critics' only complaint was that half of the essays are reprints of earlier articles. Gawande's arguments, by turns inspiring and unsettling, may cause you to see your own doctor in a whole new light.

First, as a quick proxy of how good it is, and as a way of enticing busy readers, I should note that I finished Atal Gawande's book Better: A Surgeon's Notes On Performance in less than four hours. I can't remember the last time that happened. True, it's a relatively short book, and I had some uninterrupted time on a bus. But mostly it's that Gawande is a straightforward, energetic, thoughtful writer whose essays relentlessly pull you forward. Each discusses one or two ideas in enough depth to make you realize that they're not easy problems -- which is all most people need, and which does a world of good on its own. Every country has its unquestioned assumptions; it's the rare writer who helps us question them and gently remind us that if there were easy solutions, we'd have found them by now. Gawande is good at that.

The most moving and thought-provoking of these essays, to me, was "The Doctors of the Death Chamber," in which Gawande interviews four doctors (whom he labels "A," "B," "C" and -- wait for it -- "D," in order to secure their anonymity) who help states carry out the death penalty humanely. The use of "humanely" here is questionable; it's humane in the sense that, if we are to use the death penalty, we must not be needlessly cruel at the time of the criminal's death. But it's inhumane in the larger sense that we are furthering a corrupt system -- we are "tinker[ing] with the machinery of death," to use Justice Blackmun's words. Since a doctor's role is to protect human lives, are anaesthesiologists who help execute people painlessly violating their roles? To put it more succinctly: should a doctor make the best of the machinery of death, or should he take no part in the machine? The American Medical Association has its answer and its role.
Atul Gawande is a general surgeon at the Brigham and Women's Hospital in Boston and -- from everything I've heard and read about him recently -- one of the best of the new breed of medical writers who devote their prose to informing the general public about important concerns in the world of medicine. If this new book, "Better: A Surgeon's Notes on Performance," is a representative example of his usual writing talent, I will completely agree with the above assessment. This collection of original and previously published essays is highly readable and very interesting. Normally, I am not all that interested in reading about medical topics unless it impacts me personally. I'm not a great fan of hospitals nor am I enthusiastic about going to a physician. Fortunately, for most of my life I have enjoyed relatively excellent health. My attitude, however, changed five years ago and Gawande's book takes on some genuine relevance for me. How so and why?

In a section of his book, entitled "The Mop-Up," Gawande discusses polio and the campaign to wipe it out in Asia wherein he was a momentary observer in the field in 2003. Way back in ancient history, when I was a mere child in the 1940s and America was hit with a polio epidemic, I was diagnosed with polio and almost died. Hence the relevance here for me. But more than that, I am convinced to this day that I was "saved" because of the efforts of a nurse -- I'm sure she was one of Gawande's "positive deviants" which he describes in his book -- who insisted on treating me and others with a controversial treatment (opposed by most of the medical "establishment" at the time) called "The Sister Kenny Method." She never lost a patient, by the way; we all recovered without any significant aftereffects that I know of.

This book is a fine collaborative discourse defending institutional nursing and institutional quality assurance.
The expanding and changing roles and boundaries of nursing are noted. The changing Doctor Nurse game, shared governance, evidence based practice, self directed education, and decentralization are noted. As a previous reviewer has stated, we have heard this all before.
As Dr Lawrence Savett suggests, We may learn a lot about nursing from this book. The authors suggest we need a lot of consultation so that top heavy administrations can get it right. Terrific book well documented. It could be noted that current news reports that the British are decentralizing their top heavy health administration. As a medical provider (Nurse Practitioner), I think our system seems to need real change including less administrative cost, less constraint on important care, more collaboration, more truth in advertising for pharmaceuticals, and then the "primacy of caring" may prevail.

I received an advanced reading copy of this from the publisher via Goodread's First-reads Giveaway program.

This popular science book is a broad overview of genetic and epigenetic inheritance, basically exactly what the subtitle says. The introduction oversells the the epigenetic focus (how life experience or environment can lead the changes in DNA that are not strictly sequence-based) because the majority of the book does stay within the realms of traditional sequence-based inherited genetic variation. Moreover, given Moalem's specialty, the focus is not so much on inheritance itself, nor even the specific mechanisms of inheritance.

Instead this book really comes down to these ideas: 1) There are a lot of genetic disorders. 2) Individually these disorders are often rare. 3) It is fairly likely that an given individual though will have some kind of disorder. In other words, everyone is unique; most all of us have unique rare disorders of some severity or another. The truth of this may surprise some, as may the implications: namely that any health advisories are tailored for the 'average population'. But no one is average. So not everyone can take the same amounts of medication. Eating high amounts of fat may be great for some people. Eating any fruits may be really bad for someone else. Running is good exercise for your spouse, it might give you a heart-attack, etc.

"Inheritance" thereby sweeps across a wide realm of human genetic variation, threading topics together under common themes. Moalem avoids getting bogged down into a lot of detail, making this book of greatest interest to the general public with medical interests, or those in particular who find medical anomalies interesting.

I'm a little taken aback to be the first reviewer to weigh in on Alex Perry's excellent book a full three months after its hardcover publication, but I'm honored nevertheless. Perry's book follows successful businessman Ray Chambers (he's the 'ray' in Wesray - together with ex-Treasury Secretary, William E. Simon, they were pioneers in the private equity field) in his role as United Nations Secretary-General's Special Envoy for Malaria. The co-founder of the NGO Malaria No More ('MNM'), Perry details Chambers' "ideas of enlightened self-interest" brought to bear on a problem of global - and seemingly insurmountable - scale.

The story Perry relates is of MNM's efforts to end malaria deaths by 2015. To do so, the group sets an aggressive goal to finance and distribute hundreds of millions of bednets by the end of 2010. Chambers and malaria are a good match. By all accounts, malaria is not a medical problem, it's a logistics problem. By spraying insecticides and blanketing malarial districts in bednets, experts know that they can interrupt the cycle of the disease. Applied assiduously, the effects can be immediate and dramatic.

What better agent than Ray Chambers to get this done? Chambers believed that "[I]nequality was unintelligent. Poverty didn't just hurt the poor. It hurt everyone." Others reached the same conclusion. The Bill and Melinda Gates Foundation - seen in these pages often intersecting with Chambers and team - come from the same viewpoint. But the surprise of the story is that so do corporate actors like ExxonMobil and AngloGold Ashanti.

This book is a most valuable asset to sit on the clinicians desk. It is useful in starting the search re problem side effects of medication, thinking of alternative strategies with patients where there is very limited choice. The content of the book also helps to quickly confirm for the active clinician that their recollection is correct. It deals with most of the data required by a professional working in the public service. The references are good and allied to a computer internet access to papers can greatly assist a modern clinical process seeking scientifically baked management of serious mental illnesses

I am leaving next week to start a locum tenens in Wyoming. Not having practiced psychiatry in two years, I wanted a concise psychopharmacology review book. This book is terrific. Well written and interesting. It was written by two pharmacologists and a psychiatrist from Great Britain but is totally applicable to clinical practice here in the U.S.. Highly recommended.

while the general ideas are transferable, this book is specifically for pt notes. It would have been nice if this was made a little more obvious in the product description
